Subject: Huge exception overhaul: Merged ClusterStorageException with
 PersistencyException. Replaced MembershipException with
 InvalidCollectionModification CORBA Exception. Made all predef steps throw
 more accurate exceptions when they go wrong, and let more exceptions bubble
 through from underneath.
